Do I need paid Obsidian?

No. Capture works with the free version of Obsidian, and can write to any folder of Markdown files even if you don't use Obsidian at all.

What does the free trial include?

Why does it ask for Accessibility permission?

Two reasons: so Capture can see the title and URL of whatever's frontmost (to attach as context), and so it can grab any text you've highlighted when you fire a quote capture. macOS only exposes both of those through the Accessibility API. Capture never records keystrokes or watches what you type.

Does my note leave my Mac?

No. Capture is local-only. Your notes go straight to your Markdown files on disk. There's no Capture cloud and no telemetry on note content.

How many Macs can I use it on?
